Harteraad aims to enhance the quality of life for people with heart failure or atrial fibrillation by empowering them to take control and ownership of their condition and health. To achieve this, Harteraad has systematically gathered the needs and experiences of these individuals and compiled them into two patient journeys. These journeys provide a comprehensive overview of all the significant moments, including needs and experiences, associated with their condition. The most critical needs and their corresponding solutions are now available on the Harteraad website. The patiënt journey's are intended to guide professional stakeholders in the cardiovascular field to implement patient needs in healthcare and policy.
